Due to the many changes in insurance policies, it is no longer an easy task to interpret each individual policy. Although we try our best to stay aware of these changes, it is not always possible. Therefore, we urge you, as the patient, to please check with your insurance company prior to any visit.We require payments of any copay or deductible amounts to be paid at the time your appointment is being scheduled.
Due to COVID crisis, we are trying to keep everyone safe. We will send you a link by text or email to sign for you to complete the new patient paperwork, telemedicine consent form and credit card authorization form. Your card is safely secured in a HIPPA compliant database and will only be charged if you do not cancel or reschedule within 48 hours of your appointment; that will be the full amount of the visit as allowed by your insurance provider.All forms MUST be signed electronically and submitted online.
Please plan to arrive 15 minutes before your scheduled appointment if you are to be seen in the office. Have the following available on the day of your appointment
Insurance card, personal form of ID such as driver's license
Appropriate payment option: Cash, Visa, Mastercard, Amex, HSA.
Payments are due in full at the time the service is provided in our office.
TELEMEDICINE INITIAL VISIT
In the occasion that your initial evaluation or follow-up visit will be conducted by telehealth, the information in the above paragraph holds. You will not be seen if these forms are not signed and submitted at least 48hours before your appointment. Prior to your appointment, we will send you a link by text/email. On the day of your appointment, connect to wifi and click on the link to join Dr. Tamb's virtual waiting room at least 10mins to your e-visit. Allow your browser to use your webcam and microphone when prompted. You will not be seen if operating a moving vehicle. Standard no-show policies apply.
We do not complete social security, short-term or long-term disability paperwork. We do complete FMLA paperwork on a case by case basis and we charge a fee. We also do not write letters recommending pets as emotional support or service animals or for travel. Call our office for our rates and fees for FMLA.
LATE ARRIVALS/NO SHOW POLICY
We strive to see patients at their scheduled appointment time. Therefore, if you are more than 15 minutes late, you will need to reschedule your appointment. You will be charged a non-refundable $50 missed appointment fee. Late arrivals, rescheduling, and no show up to three times for established patients are subject to termination from the practice.
At Bellevue Behavioral Solutions, we try to make it as easy as possible to set up an appointment in the shortest time possible. We do request you to take your appointment time seriously. If for any reason, you cannot keep your appointment please notify us at least 48 hours before the scheduled appointment time for new patients or the full amount which is non-refundable ( amount allowed by your insurance plan for that visit) will be charged to your credit card. For established patients, we require 24hours notice to avoid a non-refundable fee of $50.
Balances due to incorrect information supplied to the office denied claims, incomplete/ invalid/denied card transactions will be considered in default and will be forwarded to a collection agency after 60 days of 1st billing statement without notice. A 35% service fee will be added to all balances forwarded to collections.
